Immediate Postoperative Complications of Sellar Suprasellar Lesions Operated via Pterional Approach

ABSTRACT

Objective: The aim of current study is to determine the frequency of post-operative complications of  pterional approach in sellar suprasellar lesions.

Study Design: Descriptive study

Place and Duration: Neurosurgery Department of Mardan Medical Complex, Mardan KPK Pakistan for the time period from December 2020 to June 2020.

Methods: There were thirty five patients of both genders with ages 20-70 years were included. After obtaining informed written permission, the recruited patients' demographic data, including age, gender, and BMI were collected. All the patients diagnosed with sellar suprasellar lesion received pterional approach. Post-operative immediate complications among patients within a week were calculated in terms of diabetes insipidus and CSF leak. SPSS 23.0 version was used to analyze complete data.

Results: Among 35 patients, 23 (65.7%) were males and 12 (34.3%) cases were females with mean age 41.8±8.45 years and had mean BMI 23.6±3.49 kg/m2. Prolactin, hudrocortisone, triiodothyronine were the most common preoperative endocrine disorders. Mean operative time was 271.6 ±16.55 min. Frequency of completely resected patients were 29 (82.9%). Post-operative over all complications rate was 9 (25.7%) in which frequency of diabetes insipidus 6 (17.1%) was higher than that of CSF leak 3 (8.6%) cases.

Conclusion: We concluded in this study that the pterional approach was effective and useful in treatment of sellar suprasellar lesions. In patients who had the pterional technique for suprasellar lesions, diabetes insipidus was a more common postoperative consequence.
