Abstract

Aim: To test the effect of clomiphene citrate 200 mg daily for five days on ovulation induction in PCOS patients with abduction in PCOS patients with irregularlipid profile, examine the postprandial triglyceride response fora high-fat meal and Abnormal glucose tolerance test (GTT) in PCOS patient.

Study Design: a controlled clinicaltrial study.

Study Setting: Al-Batool Teaching Hospital, Mosul College of Medicine, Department of Obstetrics, and Gynecology (outpatient clinic, an infertility center).

Subjects: 50 infertile women with PCOS (25 non-obese and 25 overweight), and 25 healthy women as controls.

Main Outcome Measures: Anthropometric measurement, fasting lipid profile, blood glucose level, GTT, serum cholesterol level, high-density lipoprotein (HDL), postprandial triglyceride, hormonal examine, ultrasound. examination for ovarian.

Results: non-obese and obese PCOS females had a significantly higher (at P<0.05) waist-to-hip ratio than controls. PCOS women had higher levels of fasting and 2 hours postprandial blood glucose levels in a significant difference when P-vale less than 0.05, and postprandial HDL also significantly higher in PCOS females than control at P<0.05. HDL was significantly lower in PCOS females than controls at P-value more than 0.05. Clomiphene response was significantly in Polycystic Ovary Syndrome groups with decreased GTT 33.4% than regular blood sugar (81.8%) in women with T2NIDDM 100% were clomiphene citrate resistance. Also, clomiphene citrate resistance was high in Polycystic Ovary Syndrome patients with abnormal lipid profile (with abnormal 71-4% triglyceride resistant, in irregular cholesterol 78.2 % were resistant and PCOS women with abnormal high-density lipoprotein 67.7% were clomiphene citrate resistant

Conclusion: PCOS patients with impaired GTT, NIDDM, and irregular lipid profile had further clomiphene citrate resistance to ovulation induction than with average serum glucose and lipid profile. Also, Polycystic Ovary Syndrome patients had postprandial high cholesterol levels, which is connected to elevated waist-to-hip ratio and IRdespite obesity, and this contributes to an increase inthe risk of coronary artery disease.
